Acceptable identification is required to apply for any of the following:

• Application for Alberta Health Care
• Notice of Change of Name / Address
• Notice of Change Addition (update and/or add a family member)
• Notice of Change Deletion (update and/or delete a family member)

Alberta Health Care services are available:  Monday – Friday  10:00 am – 5:00 pm

*Proof of residency may be required
